How to Apply - 3 Steps

The following directions are for those wishing to participate in the $10,000 Model Search Contest

Step ONE: Prepare Your Picture

Your photograph is obviously going to be one of the most important elements of your application. While the model does not have to have prescription eyeglasses or be a daily eyeglass wearer, the photo must include the subject wearing glasses. Though sunglasses are permitted, we strongly recommend that the model wears regular eyeglass frames as prescription eyeglasses are our focus (no pun intended)!

Notes:

1. All photos that we are able to view are acceptable. You can use your phone, computer, tablet, camera or professional pictures if you purchased them.

2. If your glasses do not have anti-reflective coating, you may want to simply remove the lenses. We will want to see your eyes, not glare or reflection.

3. If you choose to have a background or backdrop please consider using something related. Something related to Nature/Natural, Eco-friendly, Eyes, etc.

4. Only one (1) picture per person. If more than one is sent, we will choose the first picture attached regardless of quality. Please also try to include only one person in the picture.

Step TWO: Submit you picture and information

Send an email with the following information to sales@ecoeyeco.com

1. Your legal name

2. Your preferred name (if different)

3. Your Phone Number

4. Your Home Address

5. Your Email Address

6. Your Age

7. Your Social Media Info

8. Attach your photo

9. Minors will need to attach permission from a parent or legal guardian

Step THREE: Pay the Application Fee

Simply send the email then click "here" to pay the $10 application fee. Please, include the contestants name in the notes. The Entrance fee is designed only to reduce/deter unwanted applications. Unfortunately, contests like this often solicit unwanted entries or the entry of a contestant by another person. (Ex. A friend nominates someone that does not want to enter or someone playing a practical joke). In an effort to “weed-out” or avoid such entries, we charge a minimal fee to assure each contestant is serious about the application and desire to become a model. The fee also covers the costs involved in setting up the applicants profile. Entry fees must be paid up front and can be paid with debit or credit card. As previously mentioned, while this contest is entirely run by EcoEyeCo.com and independent of The Anniston Grace Foundation, proceeds from the application fees will benefit the Foundation. 4 1 If you are under the age of 18, you will need to also provide the phone number of a parent or legal guardian.

2. Your home address should be your permanent address.

3. Please include any social media pages you own personally. For example: "www.Facebook.com/yourpagename" or twitter handle, etc. We will not post on your page. This is for verification purposes in finalist steps should you become a finalist.

4. Attach one (1) photo. Photos may be in the following formats: .png; .bmp; .eps; .pdf; .gif and .jpg or .jpeg. You may also copy and paste into Microsoft word (.doc) but the quality and resolution will likely be poor. Lastly you may also text your picture to 83EcoEyeCO or (833)263-9326 texts must include the same information. We recommend that you also follow up via email to be sure your text was received.

5. Minors (under the age of 18) will need to include additional information as follows: Parent or legal guardian Full Name. If legal guardian, we will need proof of guardianship attached. Attach a copy or picture of Parent/Guardian's Drivers License. Include contact information for verification of permission and include a note at bottom with the following statement:

I, {full name} give consent and permission for {minor's full name] to participate in EcoEyeCo.com's Model Search Contest. I further attest that I have and swear to have full authority to give this consent. Furthermore, I agree to all rules and regulations of the contest and permit EcoEyeCo.com to publish, print and share photographs of and the first name of the above named minor as it relates to the contest. Should the minor become the recipient of a prize, I agree to assume responsibility for the laws and taxes associated with winning the prize.

I have provided this consent on my own free will and swear that I am the person identified in the attached licensed (or government issued identification).
